  • Make checks payable to "Old Dogs Lacrosse".
  • Mail to: Kevin Culp, P.O. Box 434, Lewis Center, OH 43035 or email to culp@olddogslacrosse.com

 

To register your team, send the following items so they are received no later than 2/15/10. If you wish, you can scan and email the waivers and drivers license copies with the roster. Otherwise, send them by US Mail to: Kevin Culp, P.O. Box 434, Lewis Center, OH 43035.

 

  • Email the completed roster spreadsheet (click here for registration spreadsheet)
  • Include a signed waiver for each player (click here for liability waiver)
  • Include a copy of each player's drivers license as proof of age.
  • Include check payable to "Old Dogs Lacrosse" for the balance of $250.

 

Pre-Registration

  • By 12/31/09, please send a separate check of $100 for each team that you'd like in the tournament (i.e. 2 teams = 2 checks, 3 teams = 3 checks)
  • A couple groups have already asked for a second team. Right now, each of your groups has one team as a soft plan for the tournament. Hard reservations will be based on the checks received by 12/31/09.
  • For fairness, I'll start by allocating one team to each group that sends in earnest money. If all 6 spots have not been filled, I'll allocate second teams based on postmarks.
  • If you send money for a team and I can't seat them, I'll return that check to you, so make sure there's a return address.
